Every sample must be accompanied by a completed submission form. Please place the form in a separate resealable bag to prevent it from getting damp or soiled.

The total cost of testing can range from $20.00 to $100.00 for commercial production and from $20.00 to $50.00 for non-commercial production.

Please use the current forms, as all previous versions are now obsolete. Submission forms can be downloaded from the links provided below.

Clients from outside of Michigan: Please note that out-of-state sample submissions have a surcharge.

General Sample Submission Form

For plant health analyses and the identification of plants/weeds or arthropods (insects, spiders, ticks, etc.).

Depending on the client, download the appropriate nematode submission form.

MSC Soybean Cyst Nematode Submission Form

For Michigan soybean growers only. This program will end on December 31, 2025.
- Please ensure that you complete all fields above the dotted line on the MSC form. We must have the correct contact information to report results. If you desire specific recommendations regarding variety selection, indicate the SCN source of resistance in the last planted variety.
