This document is a Client Choice Food Pantry Shopping List used in client‑choice pantries, where individuals and families select foods they want and will use. The form gathers basic household and contact information, notes dietary needs, and organizes food options by key categories (fruits, vegetables, grains, soups, proteins, and other items), with a set number of choices allowed in each category. This structure supports dignity, choice, and clearer communication between clients and pantry staff.

Food pantry staff, volunteers, and community champions can download and adapt this word document for their own use.

To use the form effectively, pantries can create a master list in advance by filling in available food items on the template. The master list can then be copied for each household to select preferences ahead of distribution or posted on distribution day for families to choose from what is available. Preparing and sharing these lists before distribution helps set expectations, reduces confusion, and minimizes frustration when items run out or are unavailable.
